25 | LPA Parks arrived on Thursday December 8, 2022 to conduct a case management visit. Prior to the visit, LPA completed required COVID-19 testing protocols, and a daily self-screening questionnaire for symptoms of COVID-19 infection to affirm no COVID-19 related symptoms; LPA ensured she applied hand sanitizer before entering the facility and the following Personal Protective Equipment (PPE) was worn: N-95 mask.
LPA and Judee toured the facility, LPA noticed the following improvements: relocated nurses station to be between both client hallways, linen closet organized, all g-tube supplies are organized in supply closet. Additionally, all hallways were clear of equipment and/or boxes. All 4 clients were out of bed.
LPA obtained copy of R1's seizure log and follow-up appointment with PCP.
Exit interview. A copy of this report was left at the facility. |
