On 10/5/2023 LPA Tryon visited the facility to correct an error made by LPA on the complaint Investigation Documents dated and delivered on 9/6/2023, complaint # 59-AS-20230830082900. LPA had inadvertently attached a "Continuation" page to the wrong 9099, causing a "technical error". The results of the investigation are not changing, merely the way the document pages are attached.

Unfortunately, LPA was not able to make the corrections due to technical difficulties. LPA attempted to seek assistance with the edits, but was unable to contact assistance during this visit.

LPA will leave this document with staff today and return on another date to attempt to "correct" the "mistake."
