| Based on information gathered by the Department, the Department has concerns regarding the following:
· The use of a licensed facility for a different commercial purpose while the license remains active.
· The licensee has lost control over the property as the lease agreement submitted to the Department expired on September 30, 2017. Additionally, the lease agreement states, “to be used only for activity associated with adult day center, for a term of eighteen (12) months, beginning on the 1st day of October, 2016, and ending on the 30 September, 2017”.
· The plan of operation and facility sketch on file do not reflect the current use of space.
· Roseville Fire Department has been contact to determine if the valid fire clearance per Section 82020, corresponds to the current layout and intended use.
Licensee is advised that once clients are admitted, the ADP will be responsible for providing care and supervision within the licensed space, and all staff having contact with clients must obtain criminal record clearance.
Licensee is required to submit the following to the Department by 11/03/25:
1. Updated lease agreement reflecting the current term.
2. Updated plan of operation and facility sketch, indicating how the licensed space will be used once operational.
Copy of report emailed to licensee. Licensee to submit required documents to Bethany Mirlohi via fax or email.
