Pertinent parties stated staff assist children at various stages of toilet training. Pertinent parties stated if children are utilizing diapers, diapers are checked every two hours, or as needed, and daily reports are completed. Interviews reported documentation is inconsistent, and if a child is two years old or above, daily reports do not reflect toileting/diaper changes. Pertinent parties stated the toileting plan is part of the enrollment packet. Pertinent parties stated the child’s parents are the ones who develop the plan, without teachers.
LPA reviewed a sampling of children toileting plans. LPA observed no toileting plan completed and no tracking for toileting for a child.
Based on interviews conducted and documentation reviewed, the Department has determined the preponderance of evidence standard has been met; therefore, the above allegation is found to be SUBSTANTIATED, per California Code of Regulations, Title 22, Division 12.
During the investigation, LPA determined the missing documentation poses no health and safety risk to children in care and did not affect the overall operation of the facility. This is considered a technical violation, and no citation is being issued currently.
Appeal rights issued and discussed with licensee and their signature on this form acknowledges receipt of these rights. An exit interview was conducted, a copy of this report and Notice of Site Visit was provided to facility representative, Trudy Oliver. THIS REPORT MUST BE AVAILABLE TO THE PUBLIC FOR THREE YEARS UPON REQUEST. |
