Licensing Program Analyst (LPA) Crystal Colvin arrived at the facility unannounced for the purpose of completing the facility's Annual Inspection. LPA Colvin met with Administrator Cynthia Vizcarra and advised of the purpose of the visit, and that the Annual Inspection will be limited to Infection Control only. Below is a summary of what was observed:

Infection Control: LPA Colvin went over COVID-19 best practices for infection control and prevention with Administrator Cynthia Vizcarra, who LPA Colvin found to be successfully incorporating all aspects of the facility's Mitigation Plan. LPA Colvin observed that Community Care Licensing (CCL) does not have a copy of the facility's Mitigation Plan on file, so LPA Colvin requested for the Administrator to email her a copy later today, so that LPA Colvin can update the records. Administrator Cynthia Vizcarra reported to LPA Colvin that the facility is currently operating at 25% capacity for in-person services, and that they are continuing to provide virtual services to all clients. LPA Colvin observed that the facility is comprised of three separate buildings, which Administrator Cynthia informed LPA Colvin that clients and staff are isolating in, so there is no mixing between clients or staff of each building. Staff and clients for each building utilize a designated entrance at their building and are screened in said building in order to reduce opportunity of any possible infection to spread to others. LPA Colvin observed all staff and most clients to be wearing facial coverings. LPA Colvin additionally inquired about the staff vaccination rate, which the Administrator informed LPA Colvin is 100%. Clients have hand sanitizer available to them (with staff assistance), and the bathrooms were stocked with hand soap and paper towels. While touring the facility, LPA Colvin observed postings on the walls for cough etiquette, social distancing, and infection control.

LPA Colvin requested to view the facility's PPE supplies (gloves, masks, and sanitizer, and isolation gowns) which LPA Colvin observed to be sufficient for a 30-day supply. Administrator Cynthia Vizcarra informed LPA Colvin that each building has a small supply of PPE in addition to the main storage room which LPA Colvin observed. LPA Colvin observed a designated observation/isolation room for clients who may become ill.

LPA Colvin went over the various recommended training for facility staff with Administrator Cynthia Vizcarra in relation to COVID-19 and confirmed that staff have been trained on various aspects of infection control, recognition of symptoms of COVID-19, and donning/doffing PPE. LPA Colvin inquired as to if staff have been fit tested for N95 masks, and Administrator Cynthia Vizcarra confirmed that they have conducted fit testing for all staff. LPA Colvin was informed that although all staff are vaccinated, the facility is still conducting surveillance testing for COVID-19 for staff. LPA Colvin observed the facility to be practicing all recommendations in regards to infection control. No deficiencies or Advisory Notes were issued during today's inspection.

An exit interview was conducted with Administrator Cynthia Vizcarra and a copy of this report was provided.
