25 | Licensing Program Analyst (LPA) Samuel Lopez arrived at the facility to conduct a case management visit in response to the receipt of an Unusual Incident Report (UIR). The UIR was received by the licensing department on 5/27/2025. The UIR documented an incident that occurred on 5/22/2025, involving a child, that once they arrived at home, was observed with marks/scratches, and alleged that a teacher had hit them.
Upon arrival this date on 6/3/2025, LPA Lopez met with facility Teacher Cristina Lares/ECE Coordinator Linda Rodriguez and explained the purpose of the visit. Records were reviewed, documentation was obtained, and interviews were conducted. The following information was provided/obtained: On 5/22/2025, the child was picked up from home by a transportation service from the Palm Springs Unified School District and dropped off and signed in at the facility at 8:00am. Upon arrival at the facility, staff conducted a health check of the child and documented a mark on the child’s left arm. Per child’s parent/legal guardian, the child did not have any marks when leaving home the morning of 5/22/2025. Based on sign in and out sheets reviewed, the child was at the facility from 8:00am to 11:00am, and during that time, the concerns recorded and reported via the learning genie were, the child falling on their belly but no physical sign, and an incident where the child was throwing toys and hitting their friends/classmates. No other reports were documented of the child suffering any injuries or observed with any other marks/scratches.
Please note: On May 22, 2025, which is the day in question, Licensing Program Analyst (LPA) Samuel Lopez was present at the facility conducting an unannounced annual inspection. The inspection began at 8:18am and concluded at 10:35am. During that inspection, LPA conducted record reviews, physical plant inspection, and observed interaction between staff and children. LPA did not identify any concerns or violations of regulations.
