Licensing Program Analyst (LPA) Tricia Danielson arrived to the facility to initiate a complaint investigation into the allegations listed above. LPA met with Interim Administrator (IAD) Damion Anderson and discussed the purpose of the visit and the elements of the allegations with IAD Anderson. During today's visit, LPA toured the facility, interviewed two (2) residents, five (5) staff, and obtained copies of pertinent documents. Regarding the allegation "facility residents do not have hot water", it was alleged that memory care rooms #401, #402, and #403 have not had hot water for two (2) weeks and facility management has denied having any knowledge of this issue. Interview with IAD Anderson revealed the affected rooms are in the assisted living portion of the facility, not memory care. Anderson reported the tankless hot water heater which affects those rooms is not functioning and will require total replacement. Per Anderson, the residents in the affected rooms were offered a temporary move to another room but those offers were denied. Anderson stated in the interim, those residents are bathing in a vacant room until the water heater replacement has been completed. At 11:35 AM, LPA began measuring the hot water temperatures in rooms #401, #402, and #403. The hot water in room #401 measured 77.1 degrees Fahrenheit, in room (CONTINUED ON LIC 9099-C)

#402 it measured 75.6 degrees Fahrenheit, and in room #403 it measured 76.9 degrees Fahrenheit. LPA was able to interview two (2) of the three (3) affected residents and their statements are consistent with the information provided by Anderson. Interview with two (2) of two (2) memory care staff revealed there has been no issues or complaints with the memory care hot water. LPA also measured hot water temps in four (4) memory care rooms. All memory care unit hot water temperatures measured are within regulatory guidelines.
Regarding the allegation "facility air conditioner in disrepair", it was alleged that the air conditioning in the staff break room and in some memory care rooms is not working and the temperature is too hot for residents and staff. At 12:00 PM, LPA began measuring the air temperature in the staff break room as well as in various areas of the memory care unit. The air temperature in the staff break room measured 81.3 degrees Fahrenheit. The air temperature in the main living area of memory care measured 74.1 degrees Fahrenheit. The air temperature in the West hallway of the memory care unit measured 76.2 degrees Fahrenheit, and in the East hallway it measured 74.3 degrees Fahrenheit. In memory care units #607, #612, #615, #611, it measured 77.3, 75.5, 79.1, and 83.4 degrees Fahrenheit respectively. It should be noted that LPA measured the outside air temperature to be 91.3 degrees Fahrenheit at the time of the visit.

This agency has investigated the complaint alleging "Facility residents do not have hot water" and
"Facility air conditioner in disrepair". We have found that the complaint was unfounded, meaning that the allegations were false, could not have happened and/or are without a reasonable basis.


An exit interview was conducted with IAD Anderson and a copy of this report was provided.
