LPA met with Interim Administrator Damion Anderson and explained the purpose of the visit. LPA was informed that thirty-seven (37) residents currently reside at this facility. There were twelve (12) staff on duty during the time of the visit.
During the visit, LPA toured the inside and outside of the facility and interviewed Interim Administrator Anderson. LPA assessed the available food supply and observed that the supply exceeds the requirement of a two (2) day supply of perishable foods and a seven (7) day supply of non-perishable foods. LPA observed all facility utilities to be on and operating without issue. LPA spoke with Med Techs in Assisted Living and Memory Care and both reported no issues with obtaining the required medications for residents. Anderson reported one (1) resident has allegedly tested positive for COVID-19 while hospitalized nearby for an unrelated condition. Anderson has received no notification from the hospital of any positive test results however, he did receive a phone call notification from the resident's family. The resident in question was last present in the facility on 6/23/21 and had been previously vaccinated. Anderson has been unable to confirm the alleged positive test result but is actively attempting to do so. For precautionary measures, Anderson has requested all residents refrain from utilizing the common areas of the facility until the test results can be confirmed. Per Med Tech Dilma Lozano, thirty-six (36) of the thirty-seven (37) residents have been fully vaccinated. One (1) resident has received only the first dose of the COVID-19 vaccination.
Based on the information obtained during today's visit, there are no immediate threats to the health, safety, and welfare of the residents in care. No deficiencies were cited during today's visit. An exit interview was conducted and a copy of this report was provided to Anderson. |
