25 | Licensing Program Analyst (LPA) Tricia Danielson arrived unannounced to the facility to conduct a case management visit in conjunction with complaint 18-AS-20210907142918 to check on the health, safety, and welfare of residents in care. LPA met with Carrie MacDonald, Community Relations Director (CRD), who is acting as the facility's Administrator due to the absence of an official Administrator. LPA was informed that thirty-seven (37) residents currently reside at this facility. There were thirteen (13) staff on duty during the time of the visit.
LPA observed all facility utilities to be on and operating without issue. However, on August 18, 2021, the facility received notification of termination of water services if the balance was not paid within 10 days. CRD reported no notices were made to Licensing or residents regarding the possible discontinuance of water services. The facility has an adequate food supply and all residents have prescribed meds. LPA was informed the facility's trash services were discontinued on August 27, 2021 for non-payment. LPA observed the facility dumpster and five (5) additional regular sized trash cans to be piled high and overflowing. LPA also observed four (4) bags of trash on the ground in front of the dumpster. CRD MacDonald notified Licensee of the trash services shut off but has not received any response. CRD MacDonald last communicated with the Licensee April 11, 2021. CRD MacDonald stated she has made approximately thirty (30) email attempts to make contact with the Licensee since April 11, 2021 to no avail. CRD MacDonald also reported she sent a few text messages to the Licensee as well but still has not received a response.
Based on the information obtained during today's visit, the following deficiencies are being cited deficiency is cited per California Health & Safety Code and Code of Regulations, Title 22, Division 6, Chapter 8. An exit interview was conducted with CRD MacDonald and a copy of this report, LIC809D, and appeal rights were provided. Community Care Licensing (CCL) will send a certified copy along with emailed copies of this report, LIC809D, and appeal rights to Licensee. |
