13 | Licensing Program Analyst (LPA) Tricia Danielson arrived unannounced to the facility to conclude a complaint investigation into the allegation list above. LPA met with Community Relations Director (CRD) Carrie MacDonald and explained the purpose of the visit.
During the course of the investigation, LPA interviewed six (6) staff. Four (4) of the six (6) staff interviewed worked in the memory care portion of the facility where the allegation took place. Of those four (4) staff, three (3) reported to have observed Staff #1 (S1) speaking in an inappropriate manner to Resident #1 (R1). It was reported to LPA that R1 resided at the facility from August 17, 2020 until their passing on December 21, 2020. LPA was unable to interview R1.
Based on LPA interviews which were conducted, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. California Code Of Regulations, Title 22, Division 6 is being cited on the attached LIC 9099 D.
An exit interview was conducted with CRD MacDonald and a copy of this report was provided along with Appeal Rights. |
