13 | Licensing Program Analyst (LPA) Stephanie Williams contacted the facility to deliver findings for the above allegation via telephone due to the COVID-19 pandemic. LPA identified herself and discussed the purpose of the call with Administrator, Teresa Mapilis.
LPA interviewed Staff #1 (S1) who stated that the facility could not meet the needs of Resident #1 (R1); therefore, the facility sent an eviction notice to R1 and R1's responsible party. LPA reviewed documentation which showed that the facility provided R1 with an eviction letter explaining why the letter was being given and the 30-day eviction effective date. S1 stated that R1's responsible party came to the facility and retrieved R1's belongings before the effective eviction date. LPA attempted to interview R1; however, R1 could not recall the incident.
Based on evidence obtained during today’s visit, LPA has determined that the above allegation is UNSUBSTANTIATED; meaning that although the allegation may have happened or is valid, there is not |
