Licensing Program Analyst (LPA), Stephanie Torres, conducted an unannounced visit to the facility to deliver the findings of the investigation into the above allegations. The LPA met with Business Office Manager (BOM), Jenesa McDonald, and informed her of the purpose of her visit.

Regarding the allegation, "Facility staff failed to provide meals to resident," it was alleged facility staff sometimes forget to give Resident One (R1) a meal. The LPA initiated the investigation on January 04, 2022; staff/resident interviews were conducted, records reviewed and copies of pertinent documentation obtained. R1 was interviewed and corroborated the allegation. Staff interviews reported R1 is served meals in their bedroom for dinner time only and receives meals in the dining room for breakfast and lunch. Administrator Niebres was interviewed and reported having no knowledge of the allegation. No additional information was received to corroborate or refute the legitimacy of the violation Therefore, due to a lack of information, this allegation is deemed UNSUBSTANTIATED at this time. A finding that the complaint is unsubstantiated means that although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove the alleged

Licensing Program Analyst (LPA), Stephanie Torres, conducted an unannounced visit to the facility to deliver the findings of the investigation into the above allegation. The LPA met with Business Office Manager (BOM), Jenesa McDonald, and informed her of the purpose of her visit.

Pertaining to the allegation, "Facility staff leave wheelchair bound resident in their bed all day," it was alleged facility staff leave Resident One (R1) in their bed all day, as it takes too long to transfer R1 to their wheelchair. The LPA initiated the investigation on January 04, 2022; staff/resident interviews were conducted, records reviewed and copies of pertinent documentation obtained. Administrator Niebres was interviewed and reported having no knowledge of the allegation. Staff and resident interviews revealed R1 has been left in their bed for long periods of time due to insufficient staffing. Interviews reported R1 is a two person assist and the facility only had one staff available per shift. Therefore, based on interviews, this allegation is deemed SUBSTANTIATED.
