13 | Licensing Program Analyst (LPA), Stephanie Torres, made an unannounced visit to the facility to deliver the findings of the investigation on the above allegation. The LPA was met by Administrator, Gemma Deoso, who was informed of the purpose of the visit.
Pertaining to the allegation, "Facility failed to respond to the call light," it was alleged R1 utilized the facility signal system to request staff assistance after allegedly sustaining a fall. Staff and resident interviews were conducted; Administrator Deoso confirmed R1 did sustain a fall at the facility, which resulted in a hospitalization on February 26, 2021. Resident interviews reported R1 was observed to be calling for staff assistance, however, contact could not be made. Interviews reported Resident Two (R2) utilized the call system to request assistance, however, it was only after a third resident went in search of assistance, did staff arrive. This posed an immediate threat to the resident in care. Therefore, based on interviews and records received from the facility, this allegation is deemed SUBSTANTIATED. A finding that the complaint is substantiated means that the allegation is valid because the preponderance of the evidence standard has been met. |
