This unannounced visit conducted by Amy Goldenberg, Licensing Program Analyst (LPA), is being conducted to conclude this agency’s investigation into the complaint allegations mentioned above.

During the course of the investigation LPA reviewed staff schedules, resident roster, resident records, and conducted interviews. LPA learned the following: In regard to the allegation, " Residents sustained pressure injuries while in care", record review and interviews confirmed two (2) of (6) six residents interviewed developed pressure injuries while in care. Interviews and review of staff scheduling revealed that the allegation,"Facility insufficiently staffed", was a contributing factor to development of pressure injury due to delayed staff response for calls for incontinent care at night.

This unannounced visit conducted by Amy Goldenberg, Licensing Program Analyst (LPA), is being conducted to conclude this agency’s investigation into the complaint allegation mentioned above.

During the course of the investigation LPA reviewed staff schedules, resident roster, resident records, and conducted interviews. It is alleged that a resident was confronted by an unknown staff member accusing them of "contacting the State". Resident interview revealed the allegation to be false. Resident interviewed denied the allegation. We have found the complaint allegations is unfounded, meaning that the allegation was false, could not have happened and/or is without a reasonable basis. We have therefore dismissed the complaint. A copy of this report is being reviewed with, and furnished to the facility representative.
