On August 08, 2025, Licensing Program Analyst (LPA), Venus Mixson arrived at the facility unannounced and met with Licensee Keith Walker. LPA explained the reason for the visit was to provide findings for the complaint investigation.
On September 24, 2024, Community Care Licensing received a complaint alleging, Client's Personal Rights were violated. During the investigation, LPA conducted interviews, record reviews, and made observations pertaining to the listed allegation.
Regarding the allegation Client’s personal rights were violated, it was reported Resident #1 (R1), who attends Social Vocation Services Riverside. Additionally, it was reported that the day program called the facility and spoke with the Administrator, and informed the Administrator of R1 was being offered a job, and the Administrator stated R1 could not take the job because it would interrupt with their SSI benefits. When interviewed, the Licensee, Keith Walker denied the allegation. The Licensee stated the facility did not receive any phone calls from the day program stating there was a job available for R1.

Information obtained from an interview with the Licensee indicated there was a meeting with the Inland Regional Center; where the Licensee expressed their concerns regarding SSI benefits, the loss of SSI benefits, and Personal & Incidentals (P&I) to the residents who are employed through the day program supported jobs. According to the facility Licensee R1 in the past worked at Sun Medical. It was indicated R1 resigned due to income limitations through their SSI benefits.
Information obtained through an interview with Administrator Brian Walker did not corroborate the reported allegation. According to the Administrator the day program did not call the facility to inform any staff members of R1 or any other residents in the home being offered a job. Per the Administrator most of the residents have worked through the Basic Opportunity Program, residents worked for minimum wage which did not cause residents to exceed their income limitations through their SSI benefits. LPA confirmed R1, R2, R3, and R4 are all currently working at this time. Interviews with facility staff revealed staff recalls hearing R1 say they (R1) do not want to work because they do not want to lose their P&I funds like other residents have who are working at the day program.
According to information obtained through staff interviews the facility staff did not violate R1’s personal rights, or any of the residents’ personal rights. According to the staff their best interest is for the residents, so the residents are not put in a position of experiencing financial hardship; owing to the facility and/or the day program, both the facility and day program provide loans to residents. Staff also stated there were no concerns reported to staff by residents regarding not being allowed to work on the day program or anywhere.
Information obtained from resident interviews revealed R1 had no concerns or issues with being able to work or attend the day program. Furthermore, residents confirmed there were no concerns with facility staff violating their personal rights. LPA’s review of the records revealed all residents residing in the home attend the day program and are working.
There were no documentarians to support R1 was offered a job and did not accept the job offer. Verification through record review confirmed R1 was attending the day program and working at the time of this investigation process.
Based on interviews, record reviews, and observations, the allegation Clients Personal Rights were violated has been deemed unsubstantiated. An allegation determined unsubstantiated means although the allegation may have occurred there is not sufficient evidence to support the listed allegation.

An exit interview was conducted. A copy of this report was explained and given to Licensee, Keith Walker.
