On August 08, 2025, Licensing Program Analyst (LPA), Venus Mixson arrived at the facility unannounced and met with Licensee Keith Walker. LPA explained the reason for the visit was to provide findings for the complaint investigation.
On September 24, 2024, Community Care Licensing received a complaint alleging, Client's Personal Rights were violated. During the investigation, LPA conducted interviews, record reviews, and made observations pertaining to the listed allegation.
Regarding the allegation Client’s personal rights were violated, it was reported Resident #1 (R1), who attends Social Vocation Services Riverside. Additionally, it was reported that the day program called the facility and spoke with the Administrator, and informed the Administrator of R1 was being offered a job, and the Administrator stated R1 could not take the job because it would interrupt with their SSI benefits. When interviewed, the Licensee, Keith Walker denied the allegation. The Licensee stated the facility did not receive any phone calls from the day program stating there was a job available for R1.
