13 | On August 03, 2023, Licensing Program Analyst (LPA), Venus Mixson conducted a visit to the facility and met with Natalie Marcaida, the Administrator. The visit was conducted to provide the findings for the investigation pertaining to the listed allegations. During the investigation, LPA Mixson conducted interviews with the Administrator, the Licensee, and with the facility staff and residents.
On June 09, 2023, Community Care Licensing (CCL), received information stating the staff do not have valid training, and the facility does not provide night supervision for the residents.
Regarding the allegation, the facility staff do not have valid training, information received from interviews and the record review revealed the facility staff have received valid training, the training is provided prior to staff assignment to the facility scheduled, and training certificates were provided.
Regarding the allegation, the facility is not providing staff supervision at night, the information obtained, from staff interviews, resident interviews, and the record review, it was revealed that each shift has been covered by a facility staff member. Interviews with facility staff and residents did not corroborated the listed allegation.
Based on interviews, record reviews, and observations the allegations finding has been deemed "Unsubstantiated." An allegation finding of "unsubstantiated" means "although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur. Therefore, the outcome of the listed allegations is UNSUBSTANTIATED.
An exit interview was conducted and a copy of this report, along with the LIC 811, was provided to the Administrator.
