Licensing Program Analyst (LPA), Venus Mixson conducted a visit to the facility and met with Administrator, Natalie Marcaida. The visit was conducted to provide the findings for the investigation pertaining to the listed allegations. During the investigation, LPA Mixson conducted interviews with the Licensee, Administrator, facility staff, and residents.
On June 09, 2023, Community Care Licensing (CCL), received a complaint alleging the Facility Staff do not have valid training and facility does not provide night supervision for the residents.
Regarding the allegation, the facility staff do not have valid training. It was reported that training certificates are being forged. Information received from interviews and the record review revealed facility staff have received training. The training is provided prior to staff being assigned to tasks at the facility. Training certificates were provided to LPA for review. Certificates advised the name of the training, date, time, and duration of training. The information obtained from interviews and record review, did not corroborate the listed allegation.
Regarding the allegation, the facility is not providing staff supervision at night, it was reported that staff are not present at the facility on a particular night. The information obtained from staff and resident interviews and record review, did not corroborate the information advised. It was advised that each shift has been covered by a facility staff member. Information obtained from staff and resident interviews denied that staff is not present at night. At this time, there is not sufficient evidence to prove the listed allegations did or did not occur.
Based on the information obtained from staff and resident interviews, record reviews, and observations, the allegations have been deemed as "Unsubstantiated." An allegation finding of "unsubstantiated" means "although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur.
An exit interview was conducted, a copy of this report, and appeal rights were provided to the Administrator.
