On the date and time listed above, Licensing Program Analyst (LPA) Giselle Carbullido conducted a subsequent complaint investigation to deliver final findings. Initial visits were conducted on 06/06/25 and 06/18/25 at which time LPA conducted interviews and reviewed records. LPA met with facility representative,Jessica Quiroz , toured the facility, and took a census.
It was alleged that staff handled children in a rough manner and yelled at children during nap time. During the investigation, LPA interviewed pertinent parties, including staff and children, and reviewed records.
Staff denied yelling or mishandling children in an inappropriate manner at any time. Minimal information was provided, and LPA was unable to obtain additional details (dates, location, times). LPA attempted to review video however was informed video is not accessible after a week per facility management.
Due to conflicting information obtained from what was alleged, the evidence collected was not sufficient to substantiate or refute the above allegations. Although the allegations may have happened, or are valid,

there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are UNSUBSTANTIATED.
An exit interview was conducted, a copy of this report, appeal rights and Notice of Site Visit were provided to the facility representative, Jessica Quiroz. LPA observed the Notice of Site Visit was posted by staff. THIS REPORT MUST BE AVAILABLE TO THE PUBLIC FOR THREE YEARS.
