25 | On 11/15/2021 at 12:55pm, Licensing Program Analyst (LPA) Destinee Hogue conducted a case management inspection with Director, Wendy Serna. A case management inspection is being conducted in response to the receipt of an unusual incident report (UIR). The UIR was submitted to the Riverside Regional Office on 10/06/2021. On 10/14/2021, an inspection was conducted by LPA Hogue. During the 10/14/2021 inspection, LPA interviewed pertinent parties, reviewed facility records, and conducted facility observations. During this inspection, LPA Hogue toured the facility inside and outside, took census of preschool children present on this date, verified facility associations, and discussed the following with Director, Wendy Serna.
On or about, October 5, 2021, the facility was informed by a child's authorized representative that a staff member at the facility allegedly touched a child(ren) private parts while present in the Two's Classroom. Based on the information disclosed, the date and time of the alleged incident is unknown.
LPA Hogue interviewed pertinent parties and gathered the following information. There is conflicting information on the reported incident. The involved child(ren) was enrolled in the Pre-K classroom and according to interview with parties, the Pre-K children do not use the bathroom in the Two's Classroom. However, at the end of day when children and staff attendance is low, children will join together in the Two's classroom around 5:00pm, however according to records reviewed and interviews conducted, the suspected staff member accused of inappropriately touching a child(ren) in care, is not present at the end of day.
Based on information gathered, there appears to be no violations of Title 22 Regulations found at this time. An exit interview was conducted and a copy of this report was provided to Director Wendy Serna.
No deficiencies were cited during this inspection and a Notice of Site Visit was issued and shall remain posted for 30 days.
