On 7/19/21 at 12:35pm am, Licensing Program Analyst's (LPAs) Laura Mejorado and Samuel Lopez arrived at the facility to conduct a case management inspection. LPA toured the facility, took census and verified staff's criminal record clearances and association to facility.

On 7/16/21 Community Care Licensing Investigations Branch, Investigator Lori Wood, arrived at the facility to conduct an inspection and found an unclear adult working the front desk. When LPAs arrived on 7/19/21 another unclear adult was found working in the kitchen.

These are repeat violations, as a previous licensing report was issued on 7/14/21 giving notice of the same violations.

A Civil Penalty has been assessed during this inspection. Payment is due when billed and the check(s) or money orders shall be made payable to the “California Department of Social Services”. YOU WILL RECEIVE AN INVOICE IN THE MAIL. DO NOT SEND MONEY UNTIL YOU RECEIVE YOUR INVOICE. DO NOT SEND CASH.

An exit interview was conducted, and a copy of this report was provided to Director's on this date. Also, Director's were provided a copy of their appeal rights (LIC9058) and their signature on this form acknowledges receipt of these rights.

A copy of this report must be made available to the public for 3 years.
