32 | Regarding the allegation due to lack of supervision C1 sustained an unexplained cut to their ear and staff did not provide child’s authorized representative with an incident report regarding the injury, interviews revealed that at pick up on 8/11/2023, C1 was observed to have a fresh cut to the top of C1’s ear. Interviews revealed that on 8/11/2023, C1 was in the care of S1 with a total of 12 children when the injury was likely sustained. Interview further revealed that C1 had no incidents nor any altercations with any peers that day. It was further revealed that C1 did not cry or scream at any point of the day that would have indicated C1 got an injury. Interviews further revealed the cut was only observed when C1’s authorized representative picked C1 up and pointed it out, however, had not been observed by staff prior. Staff had been unaware of the injury and there was no incident witnessed, therefore, an incident report could not have been written. Record review revealed the observation of the cut at pick up was documented later that day and kept in CCC files.
Regarding the allegation that staff did not provide adequate food service, interviews revealed there has been no issue obtaining food or with bad food being served to the children. LPA also observed snack ready to be served, which consisted of fresh watermelon and crackers. In addition, LPA conducted a tour of the kitchen to observe the food and LPA did not observe any food to be expired or moldy.
Based on confidential interviews conducted during the investigation, C1 did sustain an unexplained cut to their ear, however, it could not be determined how or when the injury occurred or if the CCC was in violation of Title 22 Regulations. Therefore, the allegations due to lack of supervision C1 sustained an unexplained injury while in care and staff did not did not provide child’s authorized representative with incident reports are unsubstantiated. Additionally, based on conflicting statements, the allegation that staff did not provide adequate food service may have occurred, however is not supported or proven by evidence. Therefore, that allegation is unsubstantiated. An exit interview was conducted and a copy of this report and appeal rights were discussed and provided to Director Jesika Guillermo. A notice of site visit was also provided and must remain posted for 30 days. |
