25 | On this date and time, Licensing Program Analyst (LPA) Laura Mejorado arrived at the facility to conduct a case management visit in response to the receipt of an unusual incident report (UIR). The UIR was received by the licensing agency on 02/09/24. Upon arrival, LPA met with facility Director Brandie Rubalcaba and stated the purpose of the visit. Records were reviewed and interviews were conducted.
Facility self reported that on 02/09/24 a staff member was accused of mishandling a child by grabbing their arm and forcing them to sit down. Interviews disclosed the subject child and another child were hitting each other and were separated. A staff member tried redirecting the subject child to a quieter area, but the subject child threw themselves on the floor. Staff member assisted subject child by putting their arms under the subject child’s arms to help them up and guide them to the library area, once at the library area the subject child threw themselves on the floor. Staff denied the allegation. The subject child was not present during today’s inspections, however other children interviews were conducted.
Based on the information gathered and compiled during this visit no citations were issued at this time.
A notice of site visit was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
