On January 11, 2024, at 10:38 AM, Licensing Program Analyst (LPA) Anastasia Flores, arrived for the purpose of delivering the findings of the above stated allegations. On December 7, 2023, at 11:35AM, LPA conducted a health and safety inspection of the facility and no immediate health concerns were noted. Copies of pertinent evidence were gathered. LPA conducted interviews with four staff members.

On November 28, 2023, this agency received allegations that children were not allowed to use the bathroom due to insufficient staffing, children are not accorded dignity in their personal relationships with staff and others and staff provided false incident reports to child’s authorized representative. It was reported that staff were instructed to not allow children to use the bathroom due to being short staffed,during naptime. Confidential interview disclosed that the facility is often short staffed resulting in lack of help during naptime. Interview with director denied allegation that the staff are not given assistance during naptime. Interview with assistant director, stated that the staff are instructed to call the front office for help when assistance is needed for bathroom or any other reason.

Confidential interviews disclose that staff in the pre-k room are instructed to tell the children to lay back down and go to sleep. Interview with the director, denied telling the staff to have the child lay back down and hold it. Confidential interviews denied ever being told to make the children lie back down and hold it. It was reported that staff notified parents of one child that due to so many bathroom accidents the child needed to be placed in the potty-training room. Confidential interviews denied the allegation, stating it was a suggestion, not requirement, the child was new to the program and has adjusted to the program. Interview with director denied allegation of telling parents they have to move their child to another classroom, stating it is something that they work on together with parents.

Based on interviews, the allegations that children were not allowed to use the bathroom due to insufficient staffing, children are not accorded dignity in their personal relationships with staff and others, and staff provided false incident reports to child's authorized representative may have occurred, however is not supported, or proven by evidence. Therefore, the allegations are unsubstantiated.

A copy of this report, appeal rights and Notice of Site Visit were provided to Director, Cathy Knudsen.

The Notice of Site Visit must be posted for 30 days.
