25 | On December 14, 2023, at 9:53 AM, Licensing Program Analyst (LPA), Anastasia Flores, conducted a Case Management visit to address an issue resulting from the complaint investigation that was conducted on 12/07/23, (Complaint Control # 10-CC-2023112813424 and Complaint control #10-CC-20231130084124) conducted. LPA met with Director, Cathy Knudsen. During the course of the complaint Investigation, the facility cameras were reviewed, and LPAs Flores and A. Shaw observed staff #1 and staff #2, physically pushing children’s heads and bodies onto the cot during naptime between the hours of 12:15 PM to 2:30 PM, in the Jr. Preschool Classroom.
Staff have been instructed to make the children lie down on their tummies and enforce the children to stay quiet and in their cots during what is called nappy-time. LPA Flores consulted Director, Cathy Knudsen, how this is a violation of personal rights and that she should be reviewing the cameras to prevent further incidents..
The facility is being cited for California Code of Regulations, Title 22, Division 12, Chapter 1, section 101223(a)(1) Personal Rights; This is an immediate health, safety and personal rights risk to children in care.
The Facility representative shall post and provide copies of the report to parents/guardians of the children currently in care at the facility by the next business day and provide copies to the parents/guardians of newly enrolled children at the facility during the next 12 months. The licensee is to keep Acknowledgement Receipt (LIC 9224) signed by parents in each child’s file.
The Director was informed that the “Notice of Site Visit” must be posted for 30 consecutive days. Failure to post will result in Civil Penalties of $100.00. The “Notice of Site Visit” must be posted on or adjacent to the door.
An exit interview was conducted, a copy of this report, 809D, LIC9224, & appeal rights, along with a Notice of Site Visit was handed to Director, Cathy Knudsen. |
