25 | On 02/27/2025 at 1:15 PM, Licensing Program Analyst (LPA) Tiffanie Diep met with Director Tori Gayman for the purpose of an unannounced case management inspection to follow up on an Unusual Incident Report (UIR) received by the Department on 02/18/2025. The incident was reported by the facility within the required timeframe. Director guided LPA on a tour of the facility. LPA did not observe any day care children at the facility at the beginning of the inspection. All individuals subject to a criminal record review have obtained a criminal record clearance.
LPA made observations at the facility, obtained supporting documentation pertinent to the incident, and conducted interviews with pertinent individuals involved in the incident during today's visit. Interviews conducted disclosed a staff member called out late for the morning session scheduled for 6:00 AM to 8:00 AM. It was disclosed the facility attempted to secure a substitute staff member prior to the scheduled shift. Information obtained revealed the facility was unable to secure a substitute staff member until the morning session ended. Records reviewed indicated the staff member supervised 19 children alone for approximately 30 minutes between 7:25 and 7:55 AM.
Based on observations made at the facility, information obtained during interviews, and records reviewed, a deficiency is being cited on the attached LIC 809-D. LPA Tiffanie Diep informed the director, Tori Gayman, that this report dated 02/27/2025 documents one Type B citation as there was a potential risk to the safety of children in care.
An exit interview was conducted and report was reviewed with the director, Tori Gayman. A notice of site visit was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. |
