On 04/11/2023 Licensing Program Analysts (LPAs) Justin Giese and Claudia Caywood made an unannounced visit to the facility for another purpose. LPAs met with Facility Director, Jessica Salvador-Rivera and discussed the purpose of visit.

On 03/15/2023 LPA made an unannounced visit to the facility for another purpose and obtained information pertaining to a potential unusual incident regarding a child exiting a classroom. On 03/21/2023 and 03/29/2023, LPA made unannounced visits to the facility for other purposes and conducted interviewed with pertinent parties related to this unusual incident.

It was disclosed that on 02/22/2023 between the time of 09:56am to 10:09am a staff member was relieving another staff for a scheduled 10-minute break. During this time frame a child had opened the classroom door, exited the classroom, and was observed doing so by another staff in the hallway. It was stated staff in the classroom had visual observation of the child opening the door but due to their location in the classroom was unable to get to the child before they exited.

LPA interviewed Facility Director and staff involved in this incident. LPA recorded corroborating statements from all individuals involved. It was disclosed the child exited the classroom, was witnessed and stopped by staff present in the hallway and immediately returned to the classroom. Additionally, Facility Director was informed of this incident.

Based on information obtained from interviewed staff associated with this incident, there appear to be no violations of Title 22. Although the child exited the classroom, visual observation of the child exiting was maintained, and the child immediately returned the classroom.

An exit interview was conducted, LPAs provided Facility Director with a copy of this report and a notice of site visit on 04/11/2022. Notice of site visit must be displayed for the next 30 days.
