On 03/02/2026, at 09:35 AM, Licensing Program Analyst (LPA) Claudia Caywood conducted an unannounced visit to the facility for the purpose of concluding a complaint investigation. LPA met with Director, Jessica Salvador-Rivera regarding the above listed allegations, which were received on 12/09/2025. During the visit, LPA toured the facility, took census, and spoke to the Director regarding final findings.

Allegations: 1) Staff threatens daycare child(ren) 2) Staff handles daycare child(ren) in a rough manner 3)
Staff force daycare child(ren) to nap 4) Staff yelled at daycare child 5) Staff left daycare child in soiled clothing for a period of time.

During the investigation, LPA conducted interviews with all pertinent parties, reviewed documentation including children files, and toured the facility.

It was alleged that staff threaten daycare children. Interviews revealed that staff address children in a firm tone of voice in order to redirect children. All staff denied threatening daycare children.

It was alleged that staff would grab children by the armpits in a rough manner. All staff denied that children are being handled aggressively. Staff acknowledged that they sometimes need to assist the children by redirecting them but not in a rough manner.

It was alleged that staff force daycare children to nap. Staff stated that they sometimes assist the children with napping by sitting near the napping children to pat their back. All staff denied forcing daycare children to nap.

It was alleged that staff yelled at a daycare child. Staff stated they use a transition voice that is loud to get the children’s attention, but all staff denied yelling at daycare children.

It was alleged that staff left a daycare child in soiled clothing for a period of time. Staff denied leaving children in soiled clothing for five minutes or longer. Staff stated they will change the child shortly after a potty incident.

Based on interviews with all pertinent parties, conflicting information was obtained from what was alleged. Although the allegations may have happened, or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are UNSUBSTANTIATED.

An exit interview was conducted, and a copy of this report was provided to the Director, Jessica Salvador Rivera.

A Notice of Site Visit was also provided and posted which must stay posted for 30 days.

THIS REPORT MUST BE AVAILABLE TO THE PUBLIC, UPON THEIR REQUEST, FOR THREE YEARS.
