25 | Licensing Program Analysts(LPAs) Blanca Ruiz and Elyse Jones arrived at the facility on a Case Management visit to follow-up on an Unusual Incident Report submitted to the Department. At the time of visit, LPAs toured the facility, took census, and interviews were conducted with pertinent parties.
LPAs met and interim Director, Melinda Gaskin and Michelle Solorio. LPAs reviewed facility records and interview the child involved in the incident. The UIR was received by the licensing agency on 07/21/2022. It indicates that Child N.1 told his/her parent/legal guardian that Staff N.1 allegedly grabbed child's tongue leaving a small mark on the chin area. Facility conducted and internal investigation and obtained additional information that disproved the alleged incident. Child is still attending the facility and he/she was transferred to another class due to his/her age. Staff N.1 received additional training on positive child guidance and redirection.
Based on information gathered, it was disclosed that Staff N.1 did not physically touch the child nor did Staff touch the child's face or tongue. It was also disclosed that Staff N.1 was working with Staff N.2.
Staff N.2 confirmed that no physical contact between Staff N.1 and Child N.1 was observed during the date and time in question. Therefore, based on the information gathered the facility acted appropriately and no violations have been identified. An exit interview was held with Interim Site Director, Melinda Gaskin Director. A Notice of Site visit was issued and must be posted for 30 days.
