On 04/11/2023, at the time listed above, Licensing Program Analysts (LPAs) Justin Giese and Claudia Caywood made an unannounced visit to the facility for the purpose of concluding a complaint investigation. Allegations of this complaint were received by Licensing on 03/17/2023. LPAs was granted entry to the facility and met with Director, Jessica Salvador-Rivera.

The following was alleged: Lack of supervision resulted in child having wet clothing
It was alleged children play in the handwashing sink unsupervised and wet their clothing. LPA was unable to obtain supporting information pertaining to the dates, number of occurrences, classroom, or individuals involved in this allegation.

On 03/21/2023 LPA made an unannounced visit to the facility for the purpose of initiating this complaint investigation. LPA met with Facility Director, collected/reviewed documents, interviewed staff associated with the infant/toddler classrooms and made direct observations of infant program classrooms.

The facility has a total of four classrooms associated with the infant license. Two classrooms dedicated for younger infants and two classrooms dedicated for toddlers between ages 16 to 24 months old. LPA observed Toddler Classroom B as the only classroom with a child accessible hand washing sink.

As part of this investigation, LPA interviewed Facility Director and four staff members associated with Toddler Classroom B. All individuals interviewed stated staff always assist children with hand washing and deny children freely use the sink unsupervised and wet their clothing. Staff stated that due to the age of the children in care, they are learning how to wash hands and potty train. Staff stated children’s hands are washed before and after meals, restroom usage, or activities involving paints, and other supplies. Staff stated if a child soils their clothing for any circumstance, alternate clothing for children in available in the facility. Staff deny any incidents of children wetting their clothing due to lack od supervision occurring at the facility.

There was conflicting information received during the investigation from what was alleged. This agency has investigated the complaint alleging lack of supervision resulting in a child having wet clothing. Although the allegation may have happened, or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED.

A NOTICE OF SITE VISIT WAS GIVEN. LICENSEE WAS INSTRUCTED TO POSTED IT IN A PROMINENT LOCATION AT THE FACILITY. THE LICENSEE UNDERSTANDS THAT IT MUST REMAIN POSTED FOR THE NEXT 30 DAYS.



An exit interview was conducted, A copy of this report and appeal rights were given to the Licensee during this inspection on 04/11/2023.
