25 | Licensing Program Analysts (LPAs) Elyse Jones and Blanca Ruiz arrived at the facility to conduct a case management inspection in response to the receipt of an Unusual Incident Report (UIR) from the facility. The UIR was received by the licensing agency on 8/15/2022. The UIR states Child #1 walked out of his/her classroom without the three teachers present noticing and walked down the hall 10 feet to another classroom. The Staff who observed the child brought him/her inside the classroom and contacted management. Management picked Child #1 up from the classroom and took Child #1 back to the assigned classroom. During interviews it was revealed there were no other staff in the hallway with Child #1. Child #1 was alone in the hallway for approximately 1-2 minutes without supervision.
At the time of the inspection, LPAs toured the facility, took census, and met with Director, Melinda Gaskin. An interview with the Director was conducted to obtain details regarding the reported incident.
During the inspection, LPAs toured the facility, and the area where the alleged incident took place. Based on the information gathered, the following violation has been identified: 101429(a)(1) Responsibility for Providing Care and Supervision for Infants(a) In addition to Section 101229, the following shall apply: (1) Each infant shall be constantly supervised and under direct visual observation and supervision by a staff person at all times. Director admitted there was an absence of supervision and regulation was not met.
