On October 22, 2024, at 12:45 PM, Licensing Program Analyst (LPA) Anastasia Flores, arrived for the purpose of delivering the findings on the above stated allegations. On September 17, 2024, at 12:24 PM, LPA conducted a health and safety inspection, and no immediate concerns were observed. Copies of pertinent evidence were obtained, and interviews were conducted with five staff and other confidential interviews.

On September 12, 2024, our office received allegation that staff did not provide adequate supervision to day care children, staff yelled at day care child and staff falsified incidents involving day care child. Confidential interviews disclosed that staff failed to supervise child#1(C1), child #2 (C2) allowing C1, C2 to fail in the child care environment. Interview with Director denied allegation that staff are not adequately supervising children. Confidential interviews disclosed that C1 would bite peers, attempt to bite teachers without provocation. Interview with assistant director revealed that there was a behavior charts in place for C1, C2 to assist in preventing the negative behaviors.

Confidential interviews disclosed that staff yelled at C2 in the classroom. Confidential interviews revealed that staff#6 (S6) yelled at Child#2(C2) while redirecting C2 to transition to another area. Interviews with four out of four staff, denied yelling at a child unless it was to prevent a child from causing injury to themselves or others. Interviews with the director and assistant director denied allegation that the staff yell at the children in a disciplinary manner. Confidential interviews disclosed that S6 falsified an incident report dated 11/23/23. Other confidential interviews revealed that S6 did not witness the incident, but it was reported to S6 by several other students, and C2 admitted to hitting another peer on the playground on 11/23/23. Confidential records reviewed by LPA, no evidence of staff stating they witnessed the incident that was written on 11/23/23. LPA’s observation of video footage and records reviewed revealed no evidence of staff not providing adequate supervision to children in care.

Based on record review and interviews the allegation that staff did not provide adequate supervision to day care children, staff yelled at day care child, and staff falsified report incidents involving day care child, may have occurred, however is not supported or proven by evidence. Therefore, the allegations are unsubstantiated.


A copy of this report, appeal rights were reviewed and provided to Director, Marie Markham.

A Notice of Site Visit was issued. The Licensee understands that it must remain posted for the next 30 days.
