25 | On the date and time listed, Licensing Program Analyst (LPA) Perla Ordones arrived at the facility for the purposes of conducting a Case Management inspection in response to the receipt of an Unusual Incident Report (UIR) that was submitted by the facility on 07/23/2024. LPA was granted entry into the facility by Site Director Lubna Sohail. LPA explained the purpose of today's inspection, completed a facility tour, and obtained a census.
It was reported that on 07/19/2024, during dismissal time, a day-care child walked out of the classroom behind another family as they were leaving. LPA conducted interviews with pertinent parties who stated that at the time of the incident, one staff member was changing diapers while another was talking with authorized representatives. According to pertinent parties, both staff members realized the day-care child had walked out of the classroom when one of the staff asked where the day-care child was in order to change the child's diaper. Pertinent parties stated that one of the staff members inside the classroom retrieved the day-care child who was outside walking down the classroom ramp. The day-care child was brought back inside the classroom and the child's authorized representative was informed of the incident.
Per regulation 101229(a)(1), Responsibility for Providing Care and Supervision, "No child(ren) shall be left without the supervision of a teacher at any time". The facility was informed that supervision, including visual observation, is required at all times. Based on information obtained during the investigation, it has been determined that this is violation of Title 22 regulations.
