25 | On 06/30/26, Licensing Program Analyst (LPA), conducted an unannounced Case Management visit to follow up on Unusual Incident Report (UIR) that was submitted to Community Care Licensing (CCLD) on 06/18/2026, regarding an incident that took place on 06/17/2026. LPA met with Facility Director Ms. Katrina Wanemacher and informed her of the reason for the visit.
On 06/18/26, the following was reported to CCLD; on 06/17/26, at approximately 9:50 am, Preschool classroom 1 was transitioning from the classroom to the playground. During the transition, Child #1 was separated from the group. Child #1 remained in the classroom for approximately 8-10 minutes while the remaining classroom was in the playground. There were 19 children with 2 staff on the day and time of the incident. During the outdoor playtime staff #2 noticed that child#1 was not on the playground and therefore opened the door to the classroom to check and found the child in the classroom. The child was seated on the carpet waiting for their turn to go outside. As per the staff the child did look a little upset, Child #1 was accompanied to the playground and parent was notified over the phone.
During the investigation, LPA Habeebulla toured the classrooms, playground, conducted interviews, and reviewed records. Interviews and record review revealed that Child #1 was left unattended in the Preschool classroom for approximately 8 – 10 minutes.
This facility is being issued a TYPE A deficiency for Responsibility for Providing Care and Supervision with an immediate $500 civil penalty assessed. See 809D and LIC421IM for details.
An exit interview was conducted, a copy of this report, and appeal rights were given.
Exit interview conducted and report was reviewed with the Director, Katrina Wanemacher. Appeal rights were provided, and a Notice of Site Visit was given and must be posted for 30 days.
