25 | On 11/29/22 at time listed above, Licensing Program Analyst (LPA) Justin Giese made an unannounced visit to the facility for another purpose. LPA met with Assistant Director, Nichelle Dickerson and discussed purpose of visit.
On 09/01/2022 the Facility self reported and submitted an unusual incident report (UIR) to Licensing. The UIR documented an incident concerning children's personal rights. On 09/09/2022, LPAs Mejorado and Ordones made an unannounced visit to the facility to conduct a case management visit in response to the receipt of the UIR. At time of this visit, LPAs conducted interviews with pertinent parties and reviewed records. It was determined more time was needed to complete the investigation.
On 10/12/2022, Licensing received a complaint. LPA Giese made an unannounced visit to the facility on 10/17/2022 to initiate the complaint investigation. At time of visit, LPA met with Assistant Facility Director, discussed the purpose of visit, and pending allegation. It was determined the UIR submitted on 09/01/2022 corroborated persons of interest and alleged events surrounding the received complaint.
As party of this case management and complaint investigation, LPAs conducted interviews with pertinent individuals, reviewed records and made observations. Although the self reported incident may have happened, or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur. There appear to be no violations of Title 22 and the facility fulfilled its reporting requirements to Licensing.
An exit interview was conducted, LPA provided Assistant Director with a copy of this report and a notice of site visit on 11/29/2022.
