On 10/25/2023 at 11:50 AM, Licensing Program Analysts (LPAs) Claudia Caywood, Raymond Moorehead, and Licensing Program Manager (LPM) Aaron Ross made an unannounced visit to the facility for the purpose of concluding a complaint investigation. LPA met with Director, Joelle Courtney regarding the above listed allegation, which was received on 8/11/2023. LPA toured the facility, took census, and spoke to the Director regarding the complaint allegations and delivered findings.

The following was alleged: Staff left day care child unattended.

During the investigation, LPA conducted interviews with staff and other pertinent individuals. Although staff deny having knowledge of any child being left alone at the facility. Interviews revealed other pertinent individuals had direct knowledge or observed a child alone for a short period of time until they were found by a parent. Based on interviews LPA was unable to determine the length of time the child was left unattended.

Pertinent individuals whose stories corroborated concluded that a child in fact had been left alone. Based on the interviews, staff left a child unattended while transitioning from the playground to a classroom.
Every staff member shares the responsibility to care and supervise the children in care. Staff failed to provide adequate care and supervision which is a violation of Title 22 regulations. The staff did not provide care and supervision as necessary to meet the child’s needs as stated in Title 22, Responsibility for Providing Care and Supervision regulation section 101229 (a) (1).
Based on LPAs interviews and records review, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. California Code of Regulations (Title 22, Division 12), are being cited on the attached LIC9099D.

Appeal rights issued and discussed with Joelle Courtney and their signature on this form acknowledges receipt of these rights.

An exit interview was conducted. A copy of this report and Notice of Site Visit was provided to the Director, Joelle Courtney. THIS REPORT MUST BE AVAILABLE TO THE PUBLIC FOR THREE YEARS.
