25 | On 05/12/2022 at 2:00PM, a case management visit is being conducted in response to the receipt of an unusual incident report (UIR) from the facility. The UIR was received by the licensing agency on 05/09/22. It indicates that staff dragged an infant by the arm across the room.
Facility records including video footage were reviewed, and staff interviews were conducted with the Director and Regional Manager. Interviews revealed facility completed personal rights and care and supervision retraining; conducted a meeting with the parent, addressed HR policies/procedures with staff as required and scheduled an additional in-service training for staff on 05/13/2022. Based on the information gathered, the following violations have been identified:
CCR 101223(a)(1) Personal Rights
(a) The licensee shall ensure that each child is accorded the following personal rights:
(1) To be accorded dignity in his/her personal relationships with staff and other persons.

LPA, Giselle Carbullido informed facility representative Sherri Morgan that this report dated 05/12/2022document(s) 1 Type A citation(s) which shall be posted for 30 consecutive days as there is an immediate risk(s) to the health, safety, or personal rights of children in care.
Also, LPA Carbullido informed the facility representative Sherri Morgan to provide a copy of this licensing report dated 05/12/2022 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. An exit interview was conducted, appeal rights discussed, and a copy of this report and notice of site visit was provided to the facility Director, Sherri Morgan.
