On Wednesday, January 8, 2025 Licensing Program Analysts (LPAs) Jesse Gardner an Hayley McCarthy conducted an unannounced Case Management inspection to follow-up on an Unusual Incident Report (UIR) submitted to Community Care Licensing CCL on December 18, 2024. LPAs met with Project Facilitator Sarah Fisher and toured the facility. LPAs interviewed the director one staff member, and one child during this inspection.

Due to the need to gather further information, and conduct further interviews, this investigation is being extended. The facility will be made aware of the findings once they are reached.

An exit interview was held with Sarah Fisher, and a copy of this report was provided along with copies of the Appeal Rights, and Notice of Site visit. This report shall be public record for three years.
