During a previous visit and today’s visit, LPA observed appropriate ratios were maintained by staff. Interviews conducted disclosed staff conduct face-to-name checks every hour and during transitions to ensure adequate supervision. Consistent statements disclosed staff members maintain current pediatric cardiopulmonary resuscitation (CPR) and first aid certifications to assist children who may experience breathing difficulties. There were no disclosures made regarding children in care experiencing breathing difficulties. LPA did not observe staff yell or handle day care children in a rough manner. Information obtained indicated staff provide alternative activities to children when handling challenging behaviors. Interviews did not disclose consistent statements regarding witnessing staff yelling or handling day care children in a rough manner when handling challenging behaviors. LPA observed the kitchen was clean, free of litter, rubbish, and free of rodents and other vermin. LPA also observed food was stored appropriately and protected from contamination. Records reviewed revealed staff responsible for food preparation are required to maintain sanitation standards. Information obtained indicated cleaning procedures for the kitchen include daily, weekly, and monthly maintenance. Interviews did not disclose consistent statements regarding all staff engaging in food-service sanitation practices. Due to conflicting statements, it is determined there was a lack of clear corroborating evidence to support the above allegations.
Based on observations made at the facility, information obtained during interviews, and records reviewed, it is determined that the allegations could not be substantiated or dismissed. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur; therefore, the allegations are UNSUBSTANTIATED.
An exit interview was conducted and report was reviewed with the assistant director, Princess Benjamin. A notice of site visit was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. |
