25 | Licensing Program Analysts (LPAs) Blanca Ruiz and Karrene Phillips conducted a case management inspection at the facility on 12/13/2021 in response to the receipt of an unusual incident report (UIR) from the facility. The UIR was received by the licensing agency on 11/16/2021. It indicates that on 11/05/2021, a child at the facility alleged that another child had inappropriately behavior.
Upon arrival, LPA met with the director of the facility, Tracy Bierman, and stated the purpose of the inspection. Facility records were reviewed, and interviews were conducted. Ms. Bierman noted there are no cameras located inside or outside the facility. LPA obtained information that on 11/05/2021 during outside play time, Child#1 had inappropriate behavior with Child#2. None of the interviewed staff reported observing any unusual behavior or interaction between children during the day in question. In addition, none of the interviewed staff observed the incident and per facility records reviewed, no incident were documented. Records revealed Child#2 was picked up by parent at 5:05 p.m. and contacted the school shortly after as it was alleged Child#2 that Child#1 touched her on the buttocks. Assistant Director stated that on 11/06/2021, she attempted to follow up with the parent of Child#2; however the mother failed to answer and the child has not returned to the facility. During today's inspection, the LPAs attempted to contact the mother of Child#2 to obtain additional details regarding the incident, however, the mother did not answer and the call went to voicemail.
Based on the information gathered and interviews conducted with pertinent parties at the facility, no deficiencies were cited during this inspection regarding the incident at this time.
An exit interview was conducted with Director, Tracy Bierman. A Notice of Site visit was issued and must be posted for 30 days. A copy of this report must be made available to the public, at the facility site, for 3 years.
