25 | On 05/12/2023, Licensing Program Analysts (LPAs) Rachel Zeron and Taityana Benson made an unannounced visit to conduct a complaint investigation, while on visit LPAs found the following deficiencies. City of Beaumont Code Enforcement inspectors were accompanying LPAs due to the allegations on the complaint investigation 09-CC-20230428134629.
LPAs observed the downstairs bedroom to have half of the ceiling missing, exposing the attic. The LPAs observed the downstairs bathroom sink and toilet to be inoperable due to the water being shut off. The toilet was not flushed and had urine settled at the bottom of the toilet. The family room ceiling had several square shaped cutouts, exposing wires, which was a safety hazard. When questioned about the damage, Licensee disclosed that the home had water damage on the second floor of the home. Licensee indicated that she did not report the damage to CCLD and has not been able to complete the repairs. A citation for failing to notify CCLD of the alterations to existing building and grounds will be issued.
LPA informed the Licensee that prior to alterations to the building and grounds of the facility, she shall notify CCLD of proposed changes by calling the duty line, their assigned LPA and by submitting a Unusual Incident/Injury Report (LIC624B) via email/fax. Licensee indicated she was not aware that department notification was required for her interior water damage and pulling up the interior carpet.
While LPAs toured the facility, LPAs asked Licensee to escort LPAs upstairs and Licensee refused. LPAs reminded Licensee that LPAs have inspection authority and Licensee still refused. A citation and a civil penalty will be issued for inspection authority refusal.
Based on the City of Beaumont Code Enforcement Inspectors facility inspection, the facility was deemed unsafe due to substandard structure, lack of running water and interior deterioration. The City of Beaumont Code Enforcement Inspectors placed a notice on the exterior of the facility indicating the facility is unsafe, do not enter or occupy.
Licensee agrees to place the facility inactive until the facility is deemed safe and operable by Community Care Licensing and the City of Beaumont Code Enforcement/Building & Safety. Upon Licensee request, the LPAs provided the Licensee with Request For Inactive Child Care License Status (LIC9211). LPAs informed the Licensee that while the facility is inactive status, she can not care for children. CCLD will need to be informed and inspected prior to returning the facility to active status.

An exit interview was conducted, Notice of Site Visit posted, appeal rights discussed and a copy of this report was provided to the licensee.
A Non-Compliance Conference has been scheduled for Wednesday, 05/17/2023 at 10:00 a.m., to further discuss violations cited.
A copy of this report must be posted and a copy provided to every parent of every child currently enrolled, as well as all newly enrolled children for the next 12 months. An "Acknowledgement of Receipt of Licensing Reports" form (LIC 9224) must be signed by each parent and placed inside each child's file. |
