25 | On 12/23/2024 a case management visit is being conducted by Licensing Program Analyst (LPA) Susan Brewer, in response to the receipt of a self-reported unusual incident report (UIR) from the facility. The LPA was greeted by Program Lead/Director Amanda Rossie and granted entry to tour the facility inside and out. The UIR was received by the licensing agency on 12/13/2024. It indicates that during the course of outdoor play, a daycare child injured their right leg, resulting in a fracture.
LPA S. Brewer obtained facility records, documents and conducted interviews with pertinent parties which indicate that facility staff were present to supervise the daycare child when the incident took place. Pertinent parties interviewed witnessed a subject child placing their foot on the ladder while climbing when the child fell and landed onto the wood chips. The LPA inspected the outdoor play equipment which was not damaged and wood chips are under the play structure to break and cushion a child's fall. The facility followed their operational plan and emergency plan, contacted parents and provided parents with a written report. Based on information gathered, the facility acted appropriately and no violations have been identified at this time.

An exit interview was conducted and a copy of this report was provided to Program Lead/Director Amanda Rossi.
