It was alleged facility staff do not ensure that daycare child's toileting needs are being met when a subject child was observed to have soiled diapers on more than one occasion. During the investigation, the LPA obtained records, made observations and conducted interviews with pertinent parties relevant to the investigation which revealed the following: The LPA did not observe violations of staff performing duties during diaper changes while caring for children. The agency accepts preschool children requesting potty training services and there are written policy and procedures for staff performing diaper changes of preschool children. The protocols include steps for best practices in hygiene during changes in the toddler program only. Facility staff are required to read and agree to the protocols. The protocols require staff to maintain a chart. Based on record review and interview, the facility does not maintain charts for diapering in the preschool program, only in the infant and toddler program. It was also determined that the facility parent handbook and admission agreement do not mention record keeping of diaper logs for preschool children, and only requests the authorized representative provide a crib sheet, diapers and wipes for their child.
LPA interviews with pertinent parties stated diaper checks are completed on an hourly basis for preschool children. Pertinent parties interviewed did not corroborate the allegation and denied preschool children are left with soiled diapers. The LPA was unable to interview other parties pertinent to the investigation. The director denied the allegation and stated there were no diapering concerns brought to their attention. The LPA was unable to obtain specific dates, photos or documentation of the alleged incidents of a subject child left in soiled diapers. In addition, the LPA was unable to interview potential witnesses regarding the allegation due to barriers of development stages and availability. Based on LPA observations, review of records, the inability to interview other pertinent parties and conflicting statements obtained during the investigation, the allegation is determined to be unsubstantiated.
It was alleged that a staff did not provide adequate supervision resulting in a daycare child sustaining an injury caused by another child. The LPA conducted a review of records, made observations and conducted interviews with pertinent parties relevant to the investigation which revealed that on 08/29/2025 an unusual incident took place with 4 children during indoor activities. Pertinent parties interviewed described a subject staff standing in the doorway of the restroom to observe children during restroom calls while supervising children lining up in the classroom prior to transitioning the children for outdoor play. Pertinent parties stated during interviews that one subject child playing in line, fell and landed on the arm of another subject children sitting on the floor, causing the child to cry.
