25 | A case management visit is being conducted in response to the receipt of six (6) unusual incident reports (UIRs) from the facility. LPA met with Master Teacher Cynthia Osorio. LPA spoke with Center Manager Rachel Smith via the phone, to gather additional details surrounding incidents. The UIRs were received by the licensing agency and dated 12/28/21, 01/03/22, 01/18/22, 01/24/22, 01/26/22 and 02/01/22.
The UIR received on 12/28/2021 indicated that Staff #1 (S1) tested positive for COVID-19 and returned on 01/03/22. The UIR dated 01/03/22 indicated that Staff #2 (S2) tested positive for and returned on 01/07/22. The UIR dated 01/18/22 indicated that Child #1 (C1) tested positive and returned to school on 01/25/22. The UIR dated 01/24/22 indicated that Staff #3 (S3) tested positive and returned on 02/07/22. The UIR dated 01/26/22 indicated that Staff #4 (S4) tested positive and returned on 02/01/22. The UIR dated 02/01/22 indicated that Child #2 (C2) and another child were riding their bikes on the playground, when the other child suddenly ran into C2 causing the child to fall off the bike and hit the back of their head.
During today's visit, LPA toured seven rooms (#1, #2, #3, #4, #5, #6 and Toddlers) to ensure the facility is still following COVID-19 protocol. LPA observed children to be napping, so they did not have on protective face coverings; however, LPA observed the mask to be stored in their cubbies.
LPA discussed the face coverings mandate with Center Manager Rachel Smith via the phone. LPA left a copy of the California Department of Public Health guidance dated 02/07/2022 and the Center for Disease Control and Prevention COVID-19 Guidance for Operating Early Care and Education/Child Care Programs. .
An exit interview was conducted and a copy of this report was provided to facility representative Cynthia Osorio.
A Notice of Site visit was given and must remain posted for 30 days.
