On July 26, 2023, at 9:41 AM, Licensing Program Analyst’s (LPA’s), Courtnee Peebles arrived unannounced to STEPPING STONES CHILD DEVELOPMENT CENTER (CCC) and met with director, Roxanne Walker to discuss the investigative finding of the allegation listed above. On July 26, 2023, at 9:41 AM, LPA conducted a tour and census of the CCC. During the investigation, LPA conducted confidential interviews with two staff (D) and (S1).

On July 18, 2023, a complaint was received alleging Staff did not adequately supervise child while in care. Confidential interviews with staff disclosed Bunnies classroom was at recess when D was supervising the children while S1 was on break. When S1 came back C1 was not present and S1 assumed C1 went home. However, C1 was found by a parent away from the playground of the building. D has since implemented a more structured and detailed "names to faces" program form ensuring the supervision of children is present.

Based on confidential interviews conducted during the investigation, the preponderance of evidence standard has been met and the allegations that Staff did not adequately supervise children while in care has been made substantiated. A copy of this report and appeal rights were given and explained to Director Roxanne Walker. A notice of site visit was provided to and posted by Director Roxanne Walker.

Director has since implented a new names to faces document ensuring adequate supervision to children in care. Multiple teachers are to sign off on document daily.

Based on confidential interviews, interviews disclosed a gate was left open by parents and C1 was able to walk outside of the playground area and was found by a parent.
