25 | Licensing Program Analysts (LPAs) Blanca Ruiz-Silva and Samuel Lopez arrived at the facility to conduct an inspection, for a separate an unrelated issue. After touring the facility, taking a census, and verifying staff associations, LPAs found the facility to be out of compliance due to staff Myriam Romanello not being associated to the facility.
The facility was cited:
101170(e)(2)Criminal Record Clearance. All individuals subject to a criminal record review pursuant to Health and Safety Code Section 1596.871 shall prior to working, residing or volunteering in a licensed facility: (2) Request a transfer of a criminal record clearance as specified in Section 101170(f)
During today's inspection, LPAs were informed by Director that associations for staff are processed by United Families INC central office, which is located off site. LPA discovered that the staff had been associated to all other sister locations but, not this facility. LPAs informed Site Supervisor and HR staff of the discrepancy. HR staff informed LPAs that Ms. Miriam Romanello would be associate to the facility today. LPA Lopez provided information on how to submit the documentation to Community Care Licensing to expedite the transfer of the criminal record clearance. LPA spoke with Site Director, Maria Lomeli, to discuss a plan of corrections to prevent this issue from reoccurring and to avoid future deficiencies similar to this issue.
An exit interview was conducted, appeal rights discussed, and a copy of this report was provided to Site Director
A copy of this report must be made available to the public, upon their request, for 3 years.
The Notice of Site Visit visit must be posted for 30 days. Failure to keep these posted for the entire 30 days will result in an immediate $100 civil penalty for each. |
