32 | Upon LPA's entry of the facility, LPA noted 2 infants asleep in their car seats in the main living area with staff not making the immediate attempt at relocation to a crib. Additionally, upon seeing LPA, Assistant Conrado Corona picked up the car seats with infants still in them, and placed the infants in a separate room down the hall out of LPA's view. LPA followed, and later found that per the Licensee, this room (#3) is an "off-limit" area. This is a violation of Title 22. Deficiency was cited. Further, during the tour, LPA noted an infant asleep in a crib with a loose fitting sheet, as well as a blanket covering them. Deficiency cited. In a separate room, LPA noted an empty crib with a blanket draped over the edge. A technical violation was issued. During record review, Children (C1, C2, C3, C4, C5, C6, and C7) were identified as needing verification of enrollment in school. A technical violation was issued. The children's roster was not current. A technical violation was issued. Emergency Consent form (LIC627) was missing in children's files, ref #1, #2, #3, #4, #5, and #6 on the attached LIC857.
An exit interview was conducted and a copy of this report was discussed and provided with the Licensee, Veronica Santos, along with copies of the LIC811 (confidential names list, LIC857 (Children’s Record Review), LIC859 (Staff Record Review), LIC809D, and Appeal Rights.
A notice of site visit was given and must remain posted for 30 days. |
