On 09/17/2024 at 12:50 PM, Licensing Program Analyst (LPA) Jesse Gardner arrived at the facility to conduct an annual inspection as part of a compliance review. LPA toured and conducted inspection with licensee inside and out, records were reviewed, and the following was observed and/or discussed:
· Normal days and hours of operation are: Monday thru Friday 12:00 AM to 11:00 PM.

· Off-limit areas include: all bedrooms, kitchen, garage, and right side of backyard.

· The facility is licensed to have no more than 14 children as a large FCCH and is operating within the licensed capacity and appropriate ratios.


· Appropriate supervision provided during this inspection

· A working telephone is present and the current phone number is on file

· A fully charged fire extinguisher (2A:10BC) was observed.

· A smoke detector and carbon monoxide detector were present not tested during inspection due to being linked to the fire department

· All hazardous items are stored inaccessible to children in the garage

· Toxins are locked away from children in care.

· Weapons are present as stated by licensee, and upon inspection, LPA found that weapons and ammunition are not stored separately. This is a violation of Title 22, and a citation was later issued. Licensee understands all guns, weapons and ammunition must be key locked separately and made inaccessible per Title 22 Regulations

· Clean, safe and age appropriate toys were observed

· Current roster is not on file

·Facility Sketch, Emergency Disaster Plan and Notification of Parent’s Rights poster are posted

· Documentation of fire and disaster drills on file – Last drill conducted on 12/6/23. Due to a drill not being conducted within 6 months and documented, citations were issued.

· No pool at time of inspection. Licensee understands all bodies of water including ponds, above ground pools & spas, in-ground pools & spas, and some fountains must be properly covered or fenced per Title 22 Regulations. In addition, all wading pools or similar product must be emptied immediately after use and stored in an upright position.

· Verification of control of property on file

· Children’s records are complete

· Employee’s records are complete on file in office records.

· Mandated Reporter Training expires on 06/2026

· Pediatric CPR and First Aid Card expires 04/20/2026

· Health & Safety Certificate - completed on 06/29/2016


· Resident and/or staff records were reviewed and all adults who require caregiver background checks have received all required clearances and/or exemptions.

The licensee confirmed that there are no Registered Sex Offenders living in the facility and/or using the facility address for their mailing address.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Family Child Care Homes Section 102417. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. The Licensee can submit transfer forms to associate new individuals or to disassociate someone from the facility at: Associations_Disassociations858@dss.ca.gov

Licensee [or facility representative] was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

During the tour of the facility, LPA found Infant, Child #1 (C1) not sleeping in a crib, but in a bassinet. When asked, Licensee stated that C1 wont sleep in a crib, and would rather sleep in the bassinet. This is a violation of Title 22. LPA continued the tour and found another Infant, Child #2 (C2) awake, but sitting in a suspended rocking bassinet. This is also a violation of Title 22. Both violations resulted in citations. In addition, upon tour of the facility, Licensee indicated that they had firearms. Upon inspection, LPA found that the ammunition is not key locked separately from the firearms stored and locked in a safe. It should be noted that the room where the firearms are kept are in an off-limit area secured by a working/latched gate; however, this is a violation of Title 22, and a citation was issued as a result.

An exit interview was conducted, and a copy of this report was reviewed with and provided to the Licensee Maria Coronado along with copies of the LIC809D, and Appeal rights were discussed and provided during the exit interview.



A notice of site visit was given and must remain posted for 30 days.
