On the date and time listed above, Licensing Program Analyst (LPA) Giselle Carbullido conducted a subsequent complaint investigation to deliver final findings. An initial visit was conducted on 08/27/25 at which time LPA conducted interviews, made facility observations, and reviewed records. Additionally, on 08/29/2025, LPA conducted additional relevant interviews. LPA met with change of ownership, facility representative, Lucita Vaughn; toured the facility and took a census.
It was alleged that the licensee and staff are not providing a healthy environment for children and are allowing dangerous or hazardous materials to be accessible and present around children in care.
The Licensing Program Analyst (LPA) conducted a walkthrough of the facility, both indoors and outdoors. During the visit on 08/27/25, the LPA observed ongoing renovations related to a change in ownership. Debris was observed in classroom one and in outdoor areas, including tools and equipment that were accessible to children through open classroom doors and unsecured outdoor spaces. Additional observations included: An exposed electrical outlet; renovation related debris and materials present; loud and continuous equipment

noise; and old furnishing debris, and a commercial waste container located in the facility’s parking lot.
During today's visit, LPA observed renovations still in progress; areas with material debris and limitations to outdoor areas,
Interviews with relevant parties confirmed that construction began in July 2025. As a result, classrooms were shifted, access to outdoor play areas was limited due to ongoing work and debris, and noise levels increased during operating hours.
Based on interviews conducted, the Department has determined the preponderance of evidence standard has been met; therefore, the above allegations are found to be SUBSTANTIATED, per California Code of Regulations, Title 22, Division 12. See LIC9099D for deficiencies cited.
An exit interview was conducted, and appeal rights discussed. LPA provided Change of Ownership, Facility Representative, Lucita Vaughn with a copy of this report, notice of site visit and appeal rights.
