On 08/2025 at 12:15 PM Licensing Program Analyst (LPA) Susan Brewer arrived at the facility to conduct an annual inspection. LPA was greeted by Licensee Gabriela Cordova and granted entry to tour the facility inside and out. LPA reviewed records and observed and/or discussed the following: Present were the licensee, 3 adult residents and 3 minor residents, one of which is included in the census due to their age. The licensee was requested to submit updated LIC279B resident children forms.

Normal days and hours of operation: Monday-Friday 6:00AM - 6:00PM
OFF-LIMIT AREAS INCLUDE: Backyard, Garage, Room 1, Room 2, Room 3 and Master Bedroom.

The inspection consisted of reviews of the following domain: Physical Plant, Care and Supervision, Records, Facility Administration, Staffing Ratio and Capacity, Personal Rights. The inspection found the facility to be in compliance in these domains, except as noted on the LIC809D.
· The facility is NOT operating within the licensed capacity and appropriate ratios. The LPA took a census of 15 children in care. The licensee exceeded their approved licensed capacity and fire clearance for no more than 14 children in care to include their own children under age 10 years old.
· The Licensee is present in the home and has ensured that children in care are supervised.
· When temporarily absent from the home, the Licensee shall arrange for a substitute adult to care for and supervise children.
· A working telephone is present.
· A fully charged fire extinguisher (2A:10BC) was observed and tagged by the fire department and needle in the green. A smoke detector and carbon monoxide detector were present and tested by the licensee during this inspection, on 08/06/2025.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

· All hazardous items ARE NOT inaccessible, this includes detergents, cleaning compounds, medications and other items which could pose a danger to children where the licensee stores over the counter medication, hygiene products, personal cosmetic perfumes, essential oils in the restroom and hallway. In addition, the licensee stores knives and lighters in lower-level kitchen drawers which are accessible to school age children in care, due to the licensee installing rubber toddler latches on the drawer.
· Storage of poisons are NOT inaccessible to children and are NOT locked where the licensee only installed toddler latches on the cabinets, which can be easily accessible to school ages children in care.
· This is a single-story home without stairs.
· The fireplace is properly screened on today’s date.
· The licensee does have guns or weapons present as stated by the Licensee. Licensee Gabriela Cordova understands all guns, weapons and ammunition must be key locked separately and made inaccessible per Title 22 regulations.
· Home is clean and orderly, with heating and ventilation for safety and comfort.
· Clean, Safe and age-appropriate toys and equipment are present for both indoor and outdoor activities.
· Outdoor play areas are fenced and/or appropriate supervision is present.
· Verification of control of property on file by Grand Deed.
· Pediatric CPR and First Aid training on 07/18/2023; Card expires on 0718/2025.
· Health & Safety Certificate - completed on 10/15/2017; No lead.
· Mandated reporter General: 06/30/2023; AB 1207 Child Care Expired: 06/30/2025
· Fire clearance: 11/01/2017
· Documentation of fire & earthquake drills to be conducted every six months: Last drill on 07/07/2025 at 3:00 PM with 11 children present, went to the front.
· There are no bodies of water, 08/06/2025. Licensee Gabriela Cordova, understands all bodies of water including ponds, above ground pools & spas, in-ground pools & spas, and some fountains must be properly covered or fenced per Title 22 regulations. The Department must be notified before and after installation of the above types of bodies of water. In addition, all wading pools or similar product must be emptied immediately after use and stored in an upright position.
· Children’s files are Incomplete on today's date where the licensee was unable to provide proof of the LIC995A parents rights, LIC282 Affidavit and LIC9150 Notification of to authorized representatives to operate up to 14 children in care.

· Staff’s files are complete on today’s date.
· A review of staff records on 08/06/2025 indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.

Incidental Medical Services (IMS) was discussed with the licensee Gabriela Cordova. The licensee stated at this time they do not have children enrolled or clients requesting IMS services. For IMS information, see PIN 22-02-CCP. The licensee understands a plan for providing IMS must be submitted to the Department, for review prior to implementing a plan. Licensee Gabriela Cordova, was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

The Licensee can submit transfer forms to associate new individuals or to disassociate someone from your facility at: Associations_Disassociations862@dss.ca.gov . The Applicant was informed of their reporting requirements and is provided with the Regional Office’s Unusual Incident Reporting email mailbox: UnusualIncidentReportsDO09@dss.ca.gov

LPA Susan Brewer also informed Licensee Gabriela Cordova, of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

LPA Susan Brewer informed Licensee Gabriela Cordova that this report dated 08/06/2025 document(s) a Type A citation(s) which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care.

Also, LPA Susan Brewer informed the Licensee Gabriela Cordova to provide a copy of this licensing report dated 08/06/2025 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report LIC9224, or other written statement, must be placed in the child's file for verification.
