On date and time listed, Licensing Program Analysts (LPAs) Sumayya Habeebulla and Naomi Hurtado arrived at the facility to conduct an annual inspection as well as an evaluation for a capacity increase. LPA Habeebulla completed this report on LPA Hurtado's computer due to technical issues. LPA toured the facility, inside and out, records were reviewed, and the following was observed and/or discussed:

· Normal days and hours of operation are: Mon – Sun, 5 Am to 4 AM

· Off-limit areas include: 2nd Floor, Garage, Formal Living Room, Formal Dining room

· The facility is licensed to have no more than 8 children as a small FCCH and is operating within the licensed capacity and appropriate ratios. There were 3 children present upon arrival and 2 more children arrived shortly.


· Appropriate supervision was being provided during this inspection

· A working telephone is present, and the current phone number is on file

· A fully charged fire extinguisher (2A:10BC) was observed. A smoke detector and carbon monoxide detector were present and tested by the Licensee during this inspection.

· Fireplace is properly screened to prevent access by children

· All hazardous items are stored inaccessible to children.

· Fire clearance was completed on 11/21/24.

· Toxins are locked and inaccessible to children in care.

· Weapons are not present. Licensee understands all guns, weapons and ammunition must be key locked separately and made inaccessible per Title 22 Regulations

· Stairs are barricaded

· Clean, safe, and age-appropriate toys are provided

· Current roster on file

· Facility Sketch, Emergency Disaster Plan and Notification of Parent’s Rights poster are posted

· Documentation of fire and disaster drills are on file – Last drill was conducted on 11/20/24.

· No bodies of water are present at this time. Licensee understands all bodies of water including ponds, above ground pools & spas, in-ground pools & spas, and some fountains must be properly covered or fenced per Title 22 Regulations. The Department must be notified before and after installation of the above types of bodies of water. In addition, all wading pools or similar product must be emptied immediately after use and stored in an upright position.

· Verification of control of property is on file – Rental Agreement

· Children’s records are not complete

· Currently facility does not have any employees.

· Mandated Reporter Training completed on 06/25/24.

· Pediatric CPR and First Aid Card expires on 11/2025

· Health & Safety Certificate - completed on 11/11/2017.

· Resident records were reviewed and all adults who require caregiver background checks have received all required clearances and/or exemptions.

Licensee was reminded that all adults 18 and over, living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with licensee [or facility representative] and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee [facility representative] of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

The application for a large family childcare will be submitted for approval with a maximum capacity of 12. Or 14 with parent notification.

As agreed, upon by licensee Ms. Pristine Perez, the following needs to be corrected prior to receiving the License to operate a large family childcare home –
1. TB test for adult Edward Apodaca
2. TB test for S1 – Kareema Perez
3. EMSA approved Preventive Health & Safety training for Licensee Ms. Pristine Perez.
4. Pictures of the backyard after the metal plates have been removed and backyard has been cleaned up.
5. Written statement about posting the license in a prominent place.
6. Submit all the deficiency corrections by the POC due date.
7. Associate S1 to the facility on Guardian

During the exit interview, the Licensee Pristine Perez, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.

A notice of site visit was given and must remain posted for 30 days.

An exit interview was conducted, and this report was reviewed with the licensee Pristine Perez. Appeal rights were discussed and provided during the exit interview.

Based on observation, the licensee did not comply with the section cited above in ensuring the outdoor play area is free of any items that would hinder with the health & safety of the children in care. There were rusted metal plates in teh backyard inside the play house and one next to the play house which poses a potential health, safety or personal rights risk to persons in care.
